Supports and services for older people

Our Ageing and Disability Services group provides a range of supports and services for older people  which reflect consumer directed care.

  • The services available through Coast Community Connections include:
  • Case management and coordination of supports
  • Personal care such as bathing and dressing
  • Domestic assistance including laundry
  • Meal preparation
  • Shopping, social activities and transport
  • In-home respite or links to residential respite
  • Home maintenance, minor and major modifications and lawn and garden services.

Coast Community Connections can support your services through funded packaged support, either through Commonwealth Home Support Program or Home Care packages. These support packages are available via My Aged Care on 1800 200 422 or www.myagedcare.gov.au.

In addition Coast Community Connections can provide supports and services on a fee-for-service basis.

Our staff have extensive experience working with older people with a range of support needs including people with dementia, and other age related concerns, carers and older people from diverse backgrounds or who identify as LGBTI.

Our Service has achieved recognition from the Australian Aged Care Quality Agency as meeting all the Australian Home Care Standards (August 2015) and has achieved third party verification in meeting the Australian Home Care Standards through Institute for Healthy Communities Australia (April 2015).

Staff have a person centred, strengths based approach to identifying needs which reflect an individuals personal circumstances and work towards maintaining independence in the community and to support families and carers.

Coast Community Connections, Ageing and Disability Services group are also subcontractors for the Compack program and the My Aged Care Regional Assessment Service.
